Comparative Evaluation of Herbal and Conventional Mouthwashes Versus Chlorhexidine in the Management of Plaque-Induced Gingivitis: A Systematic Review

Abstract

Background: Chlorhexidine (CHX) is widely regarded as the gold standard for chemical plaque control; however, its long-term use is associated with adverse effects such as staining and taste alteration. Increasing interest in herbal alternatives has led to numerous randomized clinical trials evaluating their efficacy. Objective: To systematically evaluate the effectiveness of herbal mouthwashes compared to chlorhexidine in reducing plaque, gingival inflammation, and related clinical parameters. Methods: A systematic review was conducted following PRISMA guidelines. Randomized controlled trials comparing herbal mouthwashes with chlorhexidine were included. Databases and sources were screened, and 12 studies meeting inclusion criteria were analyzed qualitatively. Results: A total of 12 randomized clinical trials were included. Most studies demonstrated significant intra-group reductions in plaque index (PI), gingival index (GI), and bleeding parameters in both herbal and CHX groups. Intergroup comparisons showed no statistically significant differences in most studies. Some herbal agents demonstrated additional benefits such as reduced side effects and better patient compliance. Conclusion: Herbal mouthwashes show comparable efficacy to chlorhexidine in managing plaque-induced gingivitis, with fewer adverse effects, supporting their use as potential alternatives.
